United State Chemical Safety and Hazard Investigation Board
Martinez, California

 

Performed field walkdowns at the accident site; interviews with operators, maintenance workers, supervisors and refinery mangers; reviews of procedures and processes for operating and maintenance planning at the facility.  Coordinated with site management, California Occupational Safety and Health Administration and the Martinez Fire Department because of the public involvement and interest in this investigation. Participated in a public hearing of the conclusions that was presented by the United States Chemical Safety and Hazard Investigation Board.  Results were provided to Petroleum Industry organizations and the United States Occupational Safety and Health Administration.

 

 

 

United States Chemical Safety and Hazard Investigation Board

Sparks, Nevada

 

Performed independent accident investigation to determine the causes of a massive explosion at the Sierra Chemical Company in Sparks Nevada during an explosives melt-pour operation manufacturing explosive boosters for the mining industry.  The work involved extensive field walkdowns, interviews and analysis to determine a sequence of events and to identify potential causes for the explosion.   Coordinated with the United Stated Bureau of Alcohol Tobacco and Firearms, local law enforcement agencies, and the Nevada Occupational Safety and Health Administration.  Participated in a public hearing of the conclusions that was presented by the United States Chemical Safety and Hazard Investigation Board.  Results of the investigation were used to provide recommendations to the Chemical Industry and Federal Occupational Safety and Health Administration.

 

Fire and pipeline rupture event for a Petroleum Company

 

Performed independent accident investigation for a petroleum company, to determine the causes of a pipeline accident.  The work involved component failure analysis, field walkdowns at the accident site; interviews with operators, maintenance workers, supervisors and managers; reviews of procedures and processes for operating and maintenance planning at the facility.  Results were provided to senior managers.

 

Chlorine release and site evacuation incident

Idaho Falls, Idaho

 

Performed independent accident investigation for the United States Department of Energy to determine the causes of an accidental release of Chlorine gas that resulted in a site evacuation of approximately 950 people. The work involved field walkdowns at the accident site; interviews with operators, maintenance workers, supervisors and managers; reviews of procedures and processes for operating and maintenance planning at the facility.  The investigation identified deficiencies in procedures, training and supervision and oversight.  The root cause of accident was that conduct of operations requirements were not implemented in the non-nuclear safety activities at the facility.  Results were provided to management of the facility.
