Performed independent accident investigation for a petroleum company, to determine the causes of a pipeline accident. The work involved component failure analysis, field walkdowns at the accident site; interviews with operators, maintenance workers, supervisors and managers; reviews of procedures and processes for operating and maintenance planning at the facility. Results were provided to senior managers.
Chlorine release and site evacuation incident
Idaho Falls, Idaho
Performed independent accident investigation for the United States Department of Energy to determine the causes of an accidental release of Chlorine gas that resulted in a site evacuation of approximately 950 people. The work involved field walkdowns at the accident site; interviews with operators, maintenance workers, supervisors and managers; reviews of procedures and processes for operating and maintenance planning at the facility. The investigation identified deficiencies in procedures, training and supervision and oversight. The root cause of accident was that conduct of operations requirements were not implemented in the non-nuclear safety activities at the facility. Results were provided to management of the facility.
